The Healthcare Assistant will contribute to the assessment, implementation and delivery of high quality patient care within a professional team. Will demonstrate responsibility to patients, relatives and colleagues through contribution to the development and monitoring of agreed evidence based practice and to the health and social care programmes The community nursing service is located in the Doncaster Care Group, with an integrated Physical and Mental health model developing through the community services, across four localities. The service is part of a forward-thinking Organisation, which supports development and innovation at all levels to enhance the quality of patient care and service development. The unplanned community nursing team operates 7 days per week, over 24 hours. Excellent communication skills will be needed, with experience working with a community nursing team, and the ability, motivation and desire to develop current clinical knowledge and skills in accordance with the full scope of the skills map. The successful candidate will be able contribute to the assessment, implementation and delivery of high-quality patient care within a professional team. In addition, you will also demonstrate responsibility to patients, relatives and colleagues through contribution to the development and monitoring of agreed evidence-based practice and to the health and social care programmes. • Undertake agreed activities as delegated by a registered nurse, with patients / service users under the direct /In-direct supervision of the qualified staff. • Contributes to the assessment, implementation and delivery of individualised evidence based care. • To maintain accurate records in the patient’s own home or electronically, record new problems as they are identified and report all actions to nurse in charge/case manager. • The post holder is required to take personal responsibility to incorporate the 6 C vision of care into their care delivery. • Provide feedback of current patient / service user status to appropriate members of the-professional team.
